Click on the link below to email me the following information:   your name, full name and birthday of the recipient, your relationship to them, amount of the Gift Certificate (in any amount you choose), and mailing address of where you would like the Gift Certficate to be sent. Indicate how you wish to pay (Paypal or cheque).

If  paying by cheque: mail the cheque along with the above information to Susan's Kindermusik,           PO Box 21130, 640 River Street, Thunder Bay ON P7A 8A7.   

If paying by debit or credit card, I will send you an invoice from Paypal directly.
