Winter 2010 Registration Form

Class sizes are limited and are filled on a first come, first served basis until classes are full. 

If paying by cheque (to "Susan's Kindermusik"), you may pay in full OR with a $35 deposit mailed along with your printed form and second post-dated cheque for the balance dated January 15/10.  Your place will be held for one week from the time of your online submission to allow time for your cheque(s) to be mailed to P.O. Box #21130, 640 River Street, Thunder Bay ON P7A 8A7.

To use a debit or credit card, the payment in full Paypal option will automatically appear after you submit the form online. 

Payment must accompany your form. You will receive a confirmation call upon receipt of payment.

Family Tuition Discounts for children concurrently enrolled: $10 less for 2nd child; $20 less for 3rd child (except in the Familytime program). Fees include applicable taxes. Enrolled families agree to abide by studio policies. A $20 charge will be applied for NSF cheques and withdrawals before Lesson One. Please note: Our Studio is a scent-free and peanut-free environment.
